Welcome to Heather Rae Transitions. As founder, I established Heather Rae Transitions after assisting my mom in her move from family home to a condominium. Recognizing the complexities involved in such transitions I understand that moving can be both a logistical challenge and an emotional journey. As a certified mindfullness and life coach and a business owner for over 20 years, I am dedicated to making the transition as stress free as possible while providing support throughout the process. It is my pleasure to guide clients through these significant life changes; I look forward to assisting you with your transition.

Our mission is to support and simplify navigating life transitions for seniors and families. Downsizing, moving, or aging in place can be stressful. We are here to help with patience, compassion and care. We treat every family we work with as we would our own. We belong to the National Association of Senior & Special Moves Managers and are dedicated to our mission.
